We’re glad you’re here. Whether you’re establishing a new primary care relationship, seeking medical weight loss, or exploring our wellness services, we want your first visit to be smooth, comfortable, and productive. Here’s everything you need to know.
Before Your First Visit
- Schedule your appointment: Call (571) 206-8696 or book online through our Privia scheduling portal.
- Complete your patient forms: Download and complete our new patient forms in advance to save time at check-in. You can also complete them digitally through the patient portal.
- Verify your insurance: Call our office or check our Insurance page to confirm we accept your plan. Our billing team will verify your benefits before your appointment.
- Gather your records: If possible, bring or have your previous physician fax your medical records, recent lab results, and imaging reports to our office. Fax: 866-383-4386
- Prepare your medication list: Bring a complete list of all medications, vitamins, and supplements you currently take — including dosages and frequency.
What to Bring to Your First Visit
- Photo ID (driver’s license or passport)
- Insurance card(s) — front and back
- Completed new patient paperwork (if not done online)
- List of current medications and supplements
- Previous medical records, lab results, or imaging (if available)
- List of questions or concerns you’d like to discuss
- Copay or payment (we accept cash, check, and all major credit cards)
What Happens at Your First Appointment
Your first visit is typically 30–45 minutes (longer for comprehensive evaluations). You’ll meet with your provider for a thorough review of your health history, a physical examination, and a discussion of your health goals and concerns. Lab work may be ordered and can often be drawn in-office. Before you leave, you’ll have a clear care plan and your next appointment scheduled.
Arrival & Check-In
Please arrive 15 minutes before your scheduled appointment to complete check-in and any remaining paperwork. If you are running late or need to reschedule, please call us as soon as possible so we can accommodate other patients.
